Required Documentation
To apply for vehicle repair assistance, you'll need to provide the following documents. Having these ready will help us process your application efficiently.
Completed application
A fully filled-out application form with all required fields.
Valid driver's license
A current, unexpired driver's license.
Vehicle registration or title
Current vehicle registration showing ownership, or the vehicle title. If you are not the registered owner, provide proof that you are the primary authorized driver of the vehicle.
Proof of residence or employment in Lane County
A document showing you live or work in Lane County (utility bill, lease, pay stub, employer letter, etc.).
Documentation of need
Evidence supporting one or more of: current employment, job search or job offer, school enrollment, workforce development participation, or significant volunteer commitment.
Letter of need
A brief personal letter explaining why the repair is needed, how the vehicle is used, how the repair will help maintain employment, seek employment, attend school, or continue volunteer service, and why financial assistance is needed.
Repair estimate authorization
Authorization for Drive Forward Lane County to obtain a repair estimate, or an estimate from an approved repair facility.
